Photo and video consent form:

Pre- and post-operative photographs of all patients who underwent surgery in our clinic are taken; surgery from start to finish Dr. Video is recorded with Oytun idil's forehead camera (GoPro). In order to be operated, you must give your consent by signing this form. We cannot accept patients who do not allow photo and video recording for surgery. You must sign this form when you come to our clinic for surgery. It is posted here for readability only.

Informed consent forms:
